Prior to scheduling your first appointment, you are welcome to print off and fill out the new patient/client registration form below. On most occasions, we will set up your new account over the phone and it will not be necessary to bring in this form. If you would like to schedule a tour to meet our staff and view our facility before making an appointment, please feel free to do so! We encourage it. You may call our office at (316) 722-1085 and our customer service staff would be happy to assist you. Please bring your registration form with you for this visit and we will be happy to set up an account for you before you leave our facility.
